Our hearts belong to individuals with developmental disabilities. We strive to support and nurture them in any way we can. That is why we provide specialized care so they can be involved with diverse community-integrated activities that help them hone their capabilities.OUR HOMES
Moore, Sharlene’s is comprised of 3 homes. The homes are all single-family home that grace a rural southern landscape. Special touches begin with the handsome framed exteriors set off by an old-fashioned front porches with attractive views, rocking chairs and traditional southern charm. The homes are surrounded by lush, green yards. Special attention has been given to selecting furnishings typical of the style enjoyed by the individuals in their own homes through the years. Moore, Sharlene's offers the opportunity to live independently while receiving the personal care and assistance.
SERVICES AND ACTIVITITES
We go the extra mile at Moore, Sharlene’s to ease the demands of daily living while providing a variety of opportunities for an active and involved lifestyle. The services includes complete meals, daily living, transportation, housekeeping and laundry services. In-house activities such as cards, crafts, games, movies, and Bible Studies are also sure to keep our residents busy. Outings to church services, family engagements, shopping, entertainment and appointments are also provided.
PRIVACY AND INDEPENDENCE
At Moore, Sharlene’s, we realize that not everyone appreciates the same environment or activities. We have provided common areas for social interaction or for your private enjoyment. Each private bedroom is fully furnished and have enough space to accommodate a resident’s personal furniture if desired. The residents have access to a shared bathroom. Residents are encouraged to decorate their rooms to their own individual taste, filling them with all the items that will truly make Moore, Sharlene’s their own home.

We ensure that our homes maximize the comfort and quality of life of your loved ones under our care. This is why we make sure that we have clean surroundings, adequate medical supplies, and dedicated support staff who are always ready to look after your loved ones. This way, we can also maximize their safety as we tend to their overall needs while enjoying our nurturing activities.
Moore, Sharlene's PCH(Community Living Arrangement) provides services to intellectually and developmentally disabled individuals through the Department of Behavior Health and Developmental Disabilities.

PROGRAM DESCRIPTION
Moore, Sharlene's PCH provides Community Access Services to intellectual and developmentally disabled individuals. The agency is located in Bainbridge, Georgia. The program is designed to have the participants integrated in the community. Staff provides full support and guidance for the individuals when they are out in the community.
Moore, Sharlene's PCH accepts referrals for clients from Support Coordination agencies, Department of Behavior Health and Developmental Disabilities and DFACS.
Moore, Sharlene's PCH serves a variety of population, autism, Down Syndrome, wheel-chair bound, mentally intellectually disabled, developmentally disabled, medically fragile and mental health.
The age for participants to be in the program is 18 years old and older.
Moore, Sharlene's PCH is committed to assisting the participant in acquiring, retaining, or improving self-help, socialization, and adaptive skills required for active community participation and independent functioning outside the participant’s home or family home. These services are interventions in the areas of social, emotional, physical, and intellectual development and may include training in the areas of daily living skills (including leisure/recreation skills); communication training; mobility training; programming to reduce inappropriate and/or maladaptive behaviors; and training in the use of common community resources.
The emphasis of training will be on assisting the individual in increasing self help, socialization skills, skills or daily living and adaptive skills required for active community participation and independent functioning outside the participant’s home or family home. These activities include accompanying individuals to the grocery store, or eating establishments; teaching an individual how to participate in appropriate social and recreational activities; and assessing other activities of community living.
The services typically occur during the day but may also take place in the evenings and weekends.
Services are individually planned to meet the participant’s needs and preferences for active community participation. These services are provided in either community-based or facility-based settings but not in the participant’s home or family home or any other residential setting.
The intended outcome of these services is to improve the participant’s access to the community through increased skills and/or less paid supports.
STAFF RATIO FOR COMMUNITY ACCESS
Community Access Group services are provided to groups of individuals, with a staff to individual ratio of one to two or more.
The staff to individual ratio for Community Access Group services cannot exceed one (1) to ten (10).Adaptive Equipment
Individuals that utilize the use of adaptive equipment must have a physician’s order for
the equipment on file. Staff must be trained on the use of the equipment that the individuals use while at the agency.

Gait belt
Communication devices
Standing frames
Hoyer lift
Exceptional Rate
For individuals with exceptional rates, the individual must be served as outlined in the ISP. This may include, but not limited to:Extra staff
